1. What information should I have available when I call to make an appointment?

• Name

• Date of birth

• Home address

• Reason for visit

• Referring physician's name (if applicable)

• Referring physician's phone number (if applicable)

• Insurance card(s), including: the group number, ID number and copayment amount (if applicable)

• Policyholder’s name and date of birth
  (if patient is not policyholder)

2. What should I bring with me to my office visit?

• Completed registration forms, if received by mail

• Insurance card(s) – you must bring the actual cards; otherwise, we cannot bill the insurance and you will be considered self-pay

• Valid photo ID (driver’s license, state ID, work ID)

• Copay (payable by cash, check or credit card)

• All current medication bottles

• Any applicable medical reports/films from referring physician
